Support Coordination

Support coordination is designed to maximize the effectiveness of your NDIS plan, providing you with the tools and resources necessary to achieve your goals. It is a capacity-building service that aims to:

  • Enhance your understanding and utilization of your NDIS plan to achieve your objectives.

  • Facilitate connections with NDIS providers, as well as community, mainstream, and other government services.

  • Boost your confidence and abilities in managing and coordinating your support.

There are three tiers of support coordination available:

  1. Support Connection focuses on empowering you to engage with various supports, including informal, community, and funded options, to fully leverage your plan and pursue your goals.

  2. Support Coordination (Coordination of Supports) helps in developing the skills required to effectively understand and implement your plan. A support coordinator will collaborate with you to ensure a diverse range of supports are utilized, enhancing your capacity to maintain relationships, manage service delivery tasks, live independently, and engage with your community.

  3. Specialist Support Coordination offers a more intensive level of support for individuals facing complex situations, providing expert assistance in navigating challenges within your support environment and ensuring consistent service delivery.

Additionally, if you have specific goals in your NDIS plan, such as finding appropriate home and living supports, your support coordination funding can be directed towards working with a coordinator who possesses the expertise to address your unique needs.

All services provided are invoiced as per the NDIS Pricing Rules and Arrangements.
